If you are using .cue + audio as your CD audio source, your ROM file and .cue file should be named the same.
In order to activate Mode 1 in emulators you should launch the ROM file, not the .cue file.

To play on real hardware you should put the audio CD in your Sega CD then boot the ROM with the game using the flash cart of your choice.
